BTS "Chitlom" station, then look for "Erwana" exit, should be able to see it on the footbridge.  The best way is to buy the incent set inside the Erwana temple, each set includes 4 candles, 12 incent sticks and 4 flower rings.  Start with the front side, put 3 incent sticks, 1 candle and 1 flower ring to the shrine, then go clockwise direction and repeat the same.
