想買住院 / 危疾保險..

Hi,
   i am a 27-year-old man, healthy and non-smoker.
   i already have a term life insurance from hsbc, and now i want to buy 住院 / 危疾保險..
   
   i want to buy insurance that can cover the basic medical expense with reasonable charges.
    requirement:
    危疾--> 保額: >= 500K
                fixed charge
                guarantee renewal (life or > 80?)
     住院--> cover: 大房 fee
                          operation fee
                          in-patient diagnosis
                          AND all other common expense
                          **optional 住院現金
                cover 80% expense (if exceed the normal fee limits)
                guarantee renewal (life or > 80?)
                charge: fixed preferred (expensive?), non-fixed also acceptable
    could anyone give me some insurance plan suggestion?
    (of course charge not too high lor...possible for $4K-5K per year??
     if not what is the mininum?)
